This form is used specifically to evaluate the supplier in our method to verify their qualifications for taking up various works for Al Maha Petroleum Products Marketing Co. S.A.O.G. Method of evaluation is purely internal and will not be revealed to the contractor in case if the contractor is not qualified.

Notes:

1. Any certificate from third party accreditation like ISO 9000 etc.

2. Letter of appreciation from any reputed Client, Consultant or Government Organization
3. Accident / Incident reports

4. Training certificates of staff for HSE

5. List of projects completed in similar industry like ours, giving details of project’s
value and type of work

6. VAT Certificate
